In alignment with federal requirements* outlined in the 2025 Paperwork Reduction Act, 糖心Vlog视频 now provides Form 1095-C electronically for eligible employees, former employees, retirees, and other covered individuals.
Beginning with the 2025 tax year, paper copies of the form will be available only upon request.
If you are actively employed by 糖心Vlog视频 and would like an electronic copy of your 1095-C, you can access your form using the options below. Download the instructions.

You must authenticate your identity by creating an account and logging in with a username and password. Review Option 2 of the instruction guide below:
Form 1095-C is issued only for months in which an individual was enrolled in the UT SELECT Medical Plan, which is the 糖心Vlog视频's reportable self-insured health plan. If you were enrolled in UT SELECT for part of the year and UT CARE for part of the year, you may receive a Form 1095-C. The form will reflect only the months you are enrolled in UT SELECT. Enrollees in the UT CARE Medicare Advantage plan will not receive a 1095 form from Blue Cross Blue Shield or UT. Medicare will send you a Form 1095. If you need a replacement form, please contact Medicare at 1-800-MEDICARE (1-800-633-4227).Why You May or May Not Receive a Form 1095-C
